Melissa Theodore is a board-certified Nurse Practitioner with a dedicated passion for adult and geriatric care. With over 20 years of healthcare experience featuring a 15-year specialty in adult and geriatric primary care, Melissa brings a unique blend of clinical expertise, systems knowledge, and compassionate care directly to patients in their homes.

Melissa earned her Masters of Science in Nursing from Rush University and has provided care within leading healthcare systems such as Rush University Medical Center, Endeavor Health System, and Advocate Aurora Health. She’s also provided care to patients in numerous assisted living and skilled nursing facilities throughout the Chicagoland are. Over the years, she has earned a reputation for thoughtful, patient-centered care and strong clinical judgement.

In addition to her traditional clinical roles, Melissa has extensive experience delivering in-home medical care, allowing her to better understand patients’ needs within their real-life environments, and identify early changes that are often missed in office-based care. This hand on approach is a cornerstone of her practice today.

Melissa also brings over a decade of experience in the insurance and value-based care space, where she audited medical records and supported clinical strategy. This background gives her a deep understanding of how healthcare systems function, and where they fall short for patients and families.

Through InHome Primecare, Melissa now provides personalized in-home nurse practitioner services, ongoing clinical oversight, early identification of health changes, and expert care coordination. She partners closely with patients, families and providers to navigate complex medical decisions and reduce unnecessary hospitalization.

As an educator, advocate and care coordinator, Melissa is passionate about closing gaps in a fragmented healthcare system. She believes care should be proactive, accessible, and deeply personalized. She is dedicated to bringing that level of care directly into the home.
